Job Description

You will be working within our small friendly team, in a diverse and dynamic environment.  The post holder will be responsible for providing Carers with information and advice, signposting to other services and carrying out carers assessments.  S/he will work in partnership with local authority and NHS agencies, in particular mental health teams, to increase the identification and recognition of carers, as well as improve access to services available for carers.
